I can't find much on the (program) part of #9. Maybe ?? it was discussed in the sept 30 class, but the recording hasn't yet been posted. The class was mostly Q&A the first part about this exercise then it moved on...
On bb there was a question about applying the geographic portion, which doesn't apply. It does show the form found on OCLC:

looks very close to our #9.
